Online Services

GP online services allow you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to:

  • book or cancel appointments online
  • renew or order repeat prescriptions online
  • view parts of your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results

You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. More information including “how to” leaflets and videos of patients and why they are using GP online services are available at

How to sign-up

For more information, please contact reception or next time you visit the surgery, bring with you a piece of photo ID (passport, driving licence) and something with your address on like a bank statement but not a gas or electric bill. If you don’t have these, please talk to us and we will help you sign-up

Proxy Access for online patient services

Proxy access for online patient services is available to Mendip Vale patients.

'Proxy Access' is the process of allowing a 3rd party to view your online patient profile and, if required, allowing them to perform actions on your behalf e.g. booking appointments or ordering medication. This can be particularly useful for parents/children, or for care of the vulnerable elderly.

Online 'Pre-Registration' With The Practice

PreRegistrationIf you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Pre-registration Form

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct, we will also ask you to have a blood pressure reading checked on the machine at the surgery and provide a urine sample for analysis.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Welcome to Mendip Vale Medical Group

This website is currently under development.

Over the next few months the practice website will be undergoing a facelift. Thank you to all those patients who have helped by completing an evaluation of the current site. We hope that once the changes have taken place, the website will be accessible, informative and user-friendly.

Please bear with us whilst we make these changes. 


The team at Mendip Vale Medical Group look after patients across a semi-rural location in North Somerset within easy reach of Bristol. Having merged with Riverbank Medical Centre in Worle and Sunnyside Surgery in Clevedon we serve over 44,000 patients. Our Langford site is an impressive modern facility, purpose built in 2013, with great parking and equipment. Our St Georges site is a wonderful 2012 purpose built surgery with all the facilities to make the environment enjoyable. We will shortly be combining our Yatton and Congresbury surgeries into a new purpose built superb medical practice with the latest design fit for the provision of modern health care, making our estate one of the newest and brightest in the south west. Riverbank will be moving into the St Georges site which is less than a mile away during 2019. Sunnyside Surgery is well placed in Clevedon with an attached pharmacy, serving patients living in Clevedon and the surrounding villages of Tickenham, Kenn, Kingston Seymour, Walton in Gordano and Weston in Gordano. There are currently 40 GPs who are fortunate to be working with a large team  Advanced Nurse Practitioners, Paramedics and 4 Clinical Pharmacist. We have a team of nurses that are second to none, providing coil and implant fitting, chronic disease review and management alongside core nursing services provision which is underpinned by a valued team of Health Care Assistants.

All of us are enthusiastic about teaching and training, undertaking both post graduate and undergraduate teaching. We would usually have 3 or 4 trainee doctors attached to the practice at any one time. The Practice also has Student Nurse placements.

The practice is RCGP Research Ready accredited, and part of the South West Primary Care Research Network. At any one time we will be involved in and be recruiting patients for a number of studies. The practice cooperates closely with other local practices in this activity. 

We have an active Patient Participation Group which provides feedback and advice to the Practice in order to ensure we provide the best possible services to our patients.

Please familiarise yourself with some of the online services such as ordering a repeat prescription. Whatever your thoughts, be sure to let us know via our feedback function. Comments and suggestions are always a great way of helping us continue to enhance the way we look after you.
