Order Your Repeat Prescription Online

Congresbury & Yatton Surgeries

You can order a prescription in the following ways:

  • By registering for on-line patient access service via this link Patient Access

  •  in person at any of the surgeries                                                           

  •  by post, enclosing a stamped addressed envelope

  •  by fax   Congresbury: 01934 834165 Yatton: 01934 876085

  •  by taking your prescription counterfoil, with the items you require ticked, to Lloyds Pharmacy Yatton, Day Lewis Congresbury, Cohens Chemist at Yatton Surgery or Clevedon Tesco.The pharmacy will then bring your request up to the surgery for the prescription to be issued in the normal way.

  •  by ringing Lloyds Pharmacy Yatton 01934 832236 (NB not during lunch hour 13:00 to 14:00)

  •  by ringing Cohens Chemist at Yatton Surgery during their opening hours 01934 832366

    Please remember we ask for 72 hours turnaround time when ordering medications. Requests received by 12:00 will be ready for collection at the surgery by 12.00 3 days later. On Fridays, if received by 12:00, prescriptions will be ready for collection after 12:00 on Wednesdays. If you would like your prescription to be sent to a chemist, please indicate this on the form below and allow at least 3 working days before collection. If you are unable to collect your prescription and require a delivery, you must arrange this with the pharmacy directly. 

    Following guidelines from our Clinical Commissioning Group (CCG) and to ensure we reduce the chance of costly waste of medications we prescribe repeat medications for 28 days at a time. Should there be circumstances where a greater quantity is required for holiday etc. we can look at increasing this as a one off.
